DHEC’s CYSHCN program consists of a multidisciplinary team to meet your specific needs. Our care coordinators consist of Registered Nurses, Registered Dietitians, and Licensed Social Workers who are all adept at navigating the health care system and providing additional resources for you and your family.

Our team focuses on care coordination to ensure appointments are kept, referrals are initiated, needs, priorities, or concerns are addressed, and transition planning for transitioning to adult health care systems.

SC DHEC also partners with Family Connection of SC, our South Carolina Parent-to-Parent Organization, that can provide additional support and resources and support to you and your family.
